Filing DELAYED registration of Reports of Death of Filipinos who died abroad BEFORE 2020:

General Requirements:

If the NOK is presently residing in the Kingdom of Saudi Arabia

1.    Accomplished Report of Death Form (4 copies, all original signed).

ENTRIES MUST BE TYPEWRITTEN (except for the signatures) or PRINT LEGIBLY

2.    Photocopies of Passport of the deceased (data pages only)

3.    Four (4) photocopies of Death Certificate issued by the Saudi Civil Affairs authenticated by MOFA with English Translation

4.    Four (4) photocopies of Air waybill if the remains were shipped

5.    Four (4) photocopies of Embalming Certificate if the remains were shipped

6.    Four (4) photocopies of Burial Certificate with English Translation if the remains were buried

7.    Four (4) photocopies of No Objection Certificate issued by the Philippine Embassy in Riyadh

8.    Photocopies of Passport of the informant (data pages only) with proof of affinity

8.1.  If the informant is an authorized representative of the next-of-kin (NOK), the following must be submitted:

8.1.1.    original copy of notarized Special Power of Attorney from the NOK

8.1.2.    two (2) valid Government-issued IDs of NOK

9.    Accomplished Affidavit of Delayed Registration (4 copies, all original signed). ENTRIES MUST BE TYPEWRITTEN (except for the signatures) or PRINT LEGIBLY

10.  Four (4) photocopies of PSA Negative Certification of Death

*Note: Upon evaluation, additional documents may be required by the evaluating desk officer from the applicant on the requirements to determine the citizenship, identity or eligibility of the deceased for registration of death under the Philippine law; such as: PSA Birth Certificate, PSA Marriage Certificate and Government-issued Valid Identification Card.

  • Consular Fee: SAR 100.00 to be paid at the Cashier located at Multipurpose Hall of the Philippine Embassy in Riyadh, Alradeef St, Al Safarat, Riyadh or at the venue of the Embassy-on-wheels
  • Forms are downloadable online (https://riyadhpe.dfa.gov.ph/donwloads) or at Civil Registry Window.
  • Requirements are to be submitted at the Multipurpose Hall of the Philippine Embassy in Riyadh, Alradeef St, Al Safarat, Riyadh or at the venue of the Embassy-on-wheels
  • Processing period: Approximately 3 to 6 months


If the NOK or authorized representative is in the Philippines

  • Consular Fee: Equivalent of USD 25.00 to be paid in Philippine peso at the Cashier located at the 2nd floor of the DFA Office of Consular Affairs, Bradco Avenue corner Macapagal Avenue, ASEANA Business Park, Parañaque City.
  • Forms are downloadable online (https://consular.dfa.gov.ph) or at Consular Records Window.
  • Requirements are to be submitted at the Ground floor (Door 1, Window 1) of DFA Office of Consular Affairs, Bradco Avenue corner Macapagal Avenue, ASEANA Business Park, Parañaque City
  • Processing period: Approximately 3 to 6 months
