REQUIREMENTS
- Petition under oath (4 originals)
- Three (3) recent passport size photos with royal blue background
- Front view (8 copies)
- Right side view (4 copies)
- Left side view (4 copies)
- DFA authenticated PSA Birth Certificate or Report of Birth (if born abroad)
- One (1) original and
- Four (4) photocopies
- If applicant is with unmarried children below the age of 18 years, the names of the children must be indicated in the Petition and copies of their DFA authenticated PSA Birth Certificates or Reports of Birth should be submitted together with the Petition
- Valid foreign passport: original and four (4) photocopies
- Previous Philippine passport: original and four (4) photocopies
- Naturalization documents: four (4) photocopies
- DFA authenticated PSA Marriage Contract: original and four (4) photocopies
- Oath of Allegiance (personal appearance)
- Fee: $50.00 or SR200.00
